Chloroquine for treatment of covid-19 in Brazil: between science and emergency need

Introduction: Considering the pandemic scenario, caused by the new coronavirus (SARS-CoV-2), researchers from all over the world started to carry out studies of drug repositioning, which would be the fastest way to find an effective treatment. Among the evaluated drugs, are chloroquine (CQ) and hydroxychloroquine (HCQ), based on this, the objective of this study was to discuss the use of CQ/HCQ in the treatment of patients with Covid-19. Methods: This is a bibliographic review, developed on the basis of articles published in journals and documents of official organs of the Brazilian government. Development: TThe use of medications must occur in a rational manner considering their effects, although there is a provision for use in indication not provided for in the package insert, and consider proof of effectiveness or in situations where the potential benefits outweigh the risks. In the case of the use of CQ/HCQ for the treatment of covid-19, studies have pointed out risks greater than the benefits, such as the predisposition to arrhythmias due to the prolongation of the QTc interval, especially when concomitant use of azithromycin, in addition to the risk of development of retinopathy, cardiomyopathy and neuromyopathy. And yet the use of these medications has been advocated by the Brazilian authorities, based on empirical justifications. Conclusion: CQ/HQC are proven efficacy and safety drugs for other diseases and not for Covid-19. Thus, any unregistered use can be considered unsafe, representing risks to the population.
